Qualifications

• Fingerprint clearance and background check required.
• Ability to handle emotionally challenging situations.
• 1 year of general experience in case management.

Responsibilities

• Conduct screenings and assessments to develop a case plan.
• Coordinate care and services with clients and teams.
• Advocate for clients before agencies and services.
• Lead group education sessions and document progress.
• On-site work required; report to work on-site daily.
