Job summary

Volunteers of America Los Angeles is seeking a Case Manager Generalist to provide on-site case management services for low-acuity clients. You will coordinate services, complete assessments, and develop action plans with the support of an integrated care team.

Qualifications

• Must pass fingerprint clearance and background checks.
• Ability to work under stressful and potentially aggressive client situations.

Responsibilities

• Conduct screenings and assessments to collect comprehensive client information.
• Develop case plans and coordinate services to meet short- and long-term needs.
• Coordinate case conferences and monitor progress with the care team.
• Advocate for clients before agencies and community resources.
• Engage in outreach and capacity building to support client goals.
