Job summary

Volunteers of America Los Angeles is seeking a Clinical Case Manager I to provide Intensive Case Management services for clients with multiple co-morbidities. This on-site role focuses on comprehensive assessments, planning, and service linkages to support successful reentry and community integration.

The role requires at least a Masters in social work, psychology, or counseling, and relevant licensure or registration with the Board of Behavioral Sciences.

Qualifications

• Masters degree in social work, psychology, or counseling from an accredited college or university.
• Registered with the Board of Behavioral Sciences as an ASW or AMFT is required.

Responsibilities

• Conduct comprehensive screenings and assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational, and health information.
• Develop and monitor individualized case plans, coordinate service linkages, and advocate for clients with external agencies.
• Coordinate case conferences, monitor progress, and connect participants to appropriate services and natural supports.
