Job summary

Pathways Healthcare in Kingston, PA, is seeking an On Call Hospice Registered Nurse to provide in-home care to patients in the Wilkes-Barre area. This weekend/night position combines clinical autonomy with teamwork to maintain high-quality, patient-centered care.

The role requires 2+ years nursing experience, a nursing degree, and proficiency with electronic medical records. We offer mileage reimbursement, a 401(k) match, and reduced electronic documentation time to support clinicians.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

• Role: On Call Hospice Registered Nurse - Weekend/Nights
• Rate: Base Compensation + Visit Rate
• Main Office location: Kingston, PA
• Service Area: Wilkes Barre area.
• Schedule: Saturday and Sunday 8:00 pm – 8:30 am
