Job summary

Pathways Healthcare is seeking an On Call Hospice Registered Nurse for weekend night shifts in Kingston, PA, serving the Wilkes-Barre area. Schedule: Saturday and Sunday, 8:00 PM–8:30 AM, with home visits and in-home care.

You’ll administer medications, initiate and revise plans of care, and collaborate with physicians and the interdisciplinary team to ensure patient-centered outcomes. New grads welcome; geriatrics experience preferred; EMR proficiency required.

Qualifications

• 2+ years' nursing experience, preferably in geriatrics.
• New nurses welcome; training program provided.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient's needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.

• Rate: Base Compensation + Visit Rate
• Main Office location: Kingston, PA
