Job summary

Pathways Healthcare is seeking a part-time nurse to provide in-home care in Westwood, MA and surrounding areas. Schedule is Saturdays and Sundays, 8:30 am – 8:00 pm, with base compensation of $600 per weekend plus $50 per visit and $100 per admission.

Mileage reimbursement and other benefits are offered. Responsibilities include administering meds, initiating and adjusting the plan of care, triaging patients, and collaborating with physicians and the care team.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient status changes.
• Responds to incoming calls during afterhours (weekends 8:30 AM to 8:00 PM).
• Assesses patients’ needs through triage and in-home care.
• Collaborates with physicians and interdisciplinary team to contribute a strong Plan of Care.
• Responds to inquiries from patients and caregivers.
• Consults patients and caregivers before, during and after treatment.
