Pathways Healthcare is seeking a Registered Nurse - Hospice for Monday to Friday nights on-call in Concord, MA. The role offers a base compensation of $1,000 per week plus $50 per visit and $100 per admission, with a schedule of 8:00 pm–8:30 am.

Part-time position with a strong training program, mileage reimbursement, and supportive team in a patient-centered home health and hospice setting. Apply to join a recognized employer delivering compassionate care.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• New nurses welcome with training program.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsabilités

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises as patient needs change.
• Responds promptly to afterhours calls.
• Assess patient needs through triaging and in-home care.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to form a strong Plan of Care.

Job Description:

• Role: Registered Nurse - Hospice (Monday to Friday Nights On-Call)
• Rate: Base Compensation ($1000 per week) + Paid per visit ($50) and per admission ($100)
• Service Area: Concord, MA and Surrounding areas
• Schedule: Monday - Friday 8:00 pm – 8:30 am

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assess patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
