Job summary

Pathways Healthcare in Boston, MA is seeking a part-time Home Health Nurse to deliver preventive and rehabilitative care at patients' homes weekends.

You will collaborate with physicians, manage plans of care, administer meds, and triage calls, with a strong emphasis on patient-centered, dignified care.

Prerequisites include 2+ years nursing, nursing degree, BLS CPR, valid driver’s license, and EMR proficiency; compensation includes weekend base and per-visit pay.

Qualifications

• 2+ years nursing experience, preferably geriatrics.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Possess a valid driver's license and own a reliable, insured vehicle.

Responsibilities

• Initiates appropriate preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assesses patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
