Job summary

Pathways Healthcare is seeking an on-call Weekend Hospice Registered Nurse to join our team in Kingston, PA.

The role is part-time, covering Saturdays and Sundays from 8:30 AM to 8:00 PM, with base compensation plus visit rate. Experience in geriatrics and EMR proficiency are valued as you support patients at home in the Wilkes-Barre area.

Qualifications

• 2+ years nursing experience, geriatrics preferred.
• New nurses encouraged to apply and training program offered.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Working knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver’s license and own reliable, insured vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Initiates the plan of care and revises it as patient needs change.
• Responds to after-hours calls during weekends (8:30 AM–8:00 PM).
• Assessing patient needs through triaging and in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to plan care.
• Responds to inquiries and concerns from patients, caregivers, and medical team.
• Consults patients and caregivers before, during and after treatment.

Job Description:

• Role: On Call Hospice Registered Nurse - Weekend/Day Shift
• Rate: Base Compensation + Visit Rate
• Main Office location: Kingston, PA
• Service Area:Wilkes Barre area.

• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
