Job summary

Pathways Healthcare is seeking a part-time nurse to join our home health and hospice team in the Boston area. You will provide compassionate in-home care on Saturdays and Sundays from 8:30 am to 8:00 pm, with weekend base pay plus per-visit and per-admission incentives.

We value experienced nurses, especially in geriatrics, and offer a strong training program, EHR proficiency expectations, and commitment to patient-centered care. A driving license and reliable vehicle are required.

Qualifications

• 2+ years nursing experience, preferably in geriatrics.
• New nurses: training program available.
• Degree in Nursing from an accredited institution.
• Proficient with electronic medical records.
• Excellent bedside manner and communication skills.
• Knowledge of health care laws and regulations.
• Maintain current BLS CPR certification.
• Valid driver's license and own vehicle.

Responsibilities

• Initiates preventive and rehabilitative nursing procedures.
• Administers medications and treatments as prescribed by the physician.
• Implements plan of care and revises as patient needs change.
• Responds to afterhours calls (Weekends 8:30 AM–8:00 PM).
• Assesses patients and provides in-home care when needed.
• Collaborates with physicians and interdisciplinary team to support Plan of Care.
• Responds to inquiries from patients and caregivers.
• Consults patients and caregivers before, during, and after treatment.

Job Description:

• Rate: Base Compensation ($600 per weekend) + Paid per visit ($50) and per admission ($100)
• Main Office location: Boston, MA
• Service Area: Boston, Roxbury, Dorchester, Hyde Park, West Roxbury, Brighton areas
• Schedule: Saturday and Sunday 8:30 am – 8:00 pm
• Job Type: Part-time

• Initiates the plan of care and makes necessary revisions as patient status and needs change.
• Responds promptly to incoming calls during afterhours. (Weekends 8:30 AM to 8:00 PM)
• Assess patient’s needs through triaging and providing in-home care when needed.
• Collaborates with physicians, nurse practitioners and interdisciplinary team to contribute a strong Plan of Care for each patient.
• Responds to inquiries and concerns from patients, their caregivers, and their medical team.
• Consults patients and caregivers before, during and after treatment.
