Job summary

Affinity Health Management seeks an On-Call Hospice RN to respond to patient census and family needs after hours. You will provide urgent clinical and psychosocial interventions, manage pain and symptoms, and collaborate with the hospice team to maximize patient outcomes.

Two years of hospice/homecare or equivalent nursing experience are preferred. You must maintain Virginia RN licensure, drive to patient homes, and participate in continuing education and quality improvement activities.

Qualifications

• Holds Virginia RN license and nursing degree.
• Hospice/homecare or hospital med-surg experience preferred.
• Ability to perform after-hours assessments and coordinate with the care team.

Responsibilities

• Be consistently available by phone and email during shifts.
• Conduct urgent physical and psychosocial assessments after hours.
• Provide safe, skillful nursing care with pain and symptom management.
• Document care thoroughly and timely in the charting system.
• Collaborate with physicians and the IDG for continuity of care.
• Arrange DME, supplies, and medications after-hours as needed.
• Participate in mandatory meetings and ongoing professional development.
• Adhere to HIPAA and maintain patient confidentiality.
• Maintain safe driving status and transportation to patient homes.
