Job summary

Affinity Hospice Management LLC is seeking an On-Call Hospice Registered Nurse to respond to patient census and family inquiries after hours. The RN will provide urgent clinical and psychosocial interventions, pain and symptom management, and collaborate with the hospice team to maximize patient outcomes.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Maintains current unencumbered RN license in NJ.
• Preferably has hospice, homecare, hospital medical-surgical or oncology nursing experience.
• Complies with HIPAA and company policies.
• Has reliable transportation with insured vehicle.

Responsibilities

• Responds to hospice patient census and family needs during after-hours.
• Performs urgent physical and psychosocial assessments and initiates interventions.
• Delivers nursing care using the nursing process and pain/symptom management.
• Communicates with physicians and the IDG; documents care Thoroughly and timely.
• Coordinates with hospital, vendors, and community staff for after-hours needs.
• Handles after-hours admissions and participates in required meetings.
