Job summary

Affinity Hospice Management LLC seeks an On-Call Hospice RN to respond to census and referrals after hours, providing urgent clinical and psychosocial support. The role emphasizes pain and symptom management and collaboration with the hospice team to maximize patient outcomes.

Responsibilities include after-hours assessments, thorough documentation, physician communication, IDG collaboration, and arranging DME or medications as needed while ensuring HIPAA compliance and ongoing professional

Qualifications

Graduate of an accredited school of nursing.
Current unencumbered license to practice in Virginia as an RN.
Preferably two years of hospice/homecare or related nursing experience.
Must be able to drive and have reliable transportation.

Responsibilities

Responds to census/referrals outside office hours; provides urgent clinical and psychosocial interventions.
Manages pain and symptoms; collaborates with the hospice team to maximize outcomes.
Performs after-hours assessments and documents care thoroughly.
Communicates with physicians and IDG; orders as required.
Maintains HIPAA compliance and professional development.
