Job summary

Affinity Health Management seeks an On-Call Hospice RN to respond after hours for urgent patient and family needs, ensuring safe environments and symptom management. You will coordinate with physicians and the IDG to provide continuous care and report changes promptly.

This role requires a Florida RN license, nursing degree, and 2+ years of hospice/homecare experience. A valid driver's license and reliable transportation are essential to visit patients in their homes.

Qualifications

• Graduate of an accredited school of nursing.
• Current unencumbered Florida RN license.
• Two years hospice/homecare or related nursing experience preferred.
• Valid driver’s license and reliable transportation.
• Strong observation, verbal and written communication skills.

Responsibilities

• Responds to hospice patient census and family needs after hours.
• Performs urgent physical and psychosocial assessments.
• Provides pain and symptom management per nursing process.
• Documents care thoroughly and timely.
• Communicates with physicians and IDG for continuity of care.
• Collaborates with vendors for DME and medications after hours.
• May conduct after-hours hospice admissions as needed.
• Engages in ongoing professional development and training.
