Qualifications

• Graduate of an accredited nursing program and licensed RN in good standing.
• Maintains current licensure and complies with state/federal nursing regulations.
• Experience in hospice, home health, hospital medical-surgical or oncology nursing preferred.

Responsibilities

• Responds to patient census and referrals during after-hours shifts.
• Provides urgent clinical and psychosocial interventions.
• Manages pain and symptom control for patients.
• Documents care accurately and timely in medical records.
• Communicates with physicians and IDG to coordinate care.
• Assists with home visits and community outreach as needed.
