Job summary

Affinity Health Management is seeking a dedicated Hospice On Call RN in Florida. The role requires responding to patient census and family needs during after-hours, providing urgent clinical and psychosocial support, and coordinating care with the hospice team and physicians.

A Florida RN license and relevant nursing experience are essential. The on-call nurse will assist with pain and symptom management, documentation, and collaboration with the IDG to maximize patient outcomes and continuity

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted Florida RN license.
• Preferably 2 years hospice/homecare or related nursing experience.

Responsibilities

• Respond to after-hours census and referrals; provide urgent clinical and psychosocial interventions.
• Conduct urgent physical and psychosocial assessments during after-hours and determine interventions.
• Provide pain and symptom management and maintain a safe environment for patients.
• Document care thoroughly; submit accurate clinical documentation.
• Coordinate with physicians and IDG; obtain orders as required.
• Collaborate with hospital/community staff to adjust plans of care.
