Job summary

Qualifications

• Graduate of an accredited school of nursing.
• Maintains current unencumbered RN license.
• Two years hospice or homecare experience preferred.
• Familiar with nursing documentation and HIPAA requirements.

Responsibilities

• Respond to after-hours census and referrals.
• Provide urgent clinical and psychosocial interventions.
• Pain and symptom management.
• Document care thoroughly and timely.
• Collaborate with physicians and IDG members.
• Coordinate after-hours admissions and transports.
