Job summary

Affinity Hospice Management LLC is seeking an experienced On-Call Registered Nurse to respond to hospice patient census and family needs outside office hours, including every other weekend. The role emphasizes urgent clinical and psychosocial interventions, pain control, and collaboration with the hospice team to maximize patient outcomes.

Responsibilities include after-hours assessments, documentation, and liaison with physicians and the IDG to ensure continuity of care.

Qualifications

• Current Florida RN license in good standing.
• Graduate of an accredited school of nursing.
• Preferred: two years hospice/homecare or related nursing experience.
• Must have valid driver’s license with insured vehicle.
• Ability to work after hours and visit patient homes.

Responsibilities

• Be consistently available by telephone and email during shifts.
• Conduct urgent physical and psychosocial assessments after hours.
• Provide safe environment and skilled nursing care.
• Manage pain and symptom control; document changes.
• Communicate with physicians; obtain orders as needed.
• Work with IDG for continuity of care and support.
• Collaborate with vendors for DME, supplies and meds after hours.
• Participate in mandatory meetings and maintain ongoing education.
• Perform additional duties as assigned.
