Job summary

One Community Hospice seeks an On Call RN to respond to after-hours patient census and family inquiries. The role provides urgent clinical and psychosocial interventions, skilled pain and symptom management, and collaborates with the hospice team to maximize patient outcomes.

The position requires a Virginia RN license and hospice/home health experience. Independent judgment in crises, thorough documentation, and driving to patient homes are essential aspects of the role.

Qualifications

• Graduate of an accredited school of nursing and licensed to practice in Virginia.
• Experience in hospice, home health, or related nursing settings preferred.
• Ability to perform nursing duties with compliance to laws and policies.

Responsibilities

• Responds to after-hours patient census and family referrals via phone and email.
• Performs urgent physical and psychosocial assessments and determines interventions.
• Maintains a safe patient environment and delivers nursing care per the nursing process.
• Provides pain control and symptom management and guides families on care plans.
• Documents patient care thoroughly and timely with changes/outcomes noted.
• Communicates with physicians and IDG members about patient needs and orders.
• Drives care coordination with vendors for supplies and meds after hours.
• Coordinates with hospital and community staff to adjust plans of care.
• Conducts after-hours hospice admissions as necessary.
• Attends required meetings and pursues ongoing professional development.
• Completes background checks and references as required.
