Job summary

Amedisys, part of the Optum family, delivers home health, hospice and community care, helping patients receive quality care at home. This Hospice RN On-Call role offers a $7,500 sign-on bonus and on-call schedule to support after-hours clinical needs.

You will provide remote and field-based nursing support, triage patient needs, coordinate care with interdisciplinary teams, and document all clinical decisions to ensure continuity and quality outcomes.

Qualifications

• Current RN license in the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and in-person visits and escalate when needed.
• Coordinate with physicians, care teams, and caregivers for timely and effective care.
• Educate patients and caregivers on symptom management and care plans.
• Maintain accurate documentation of all patient interactions and clinical decisions.
