Job summary

Amedisys is seeking an experienced Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care to ensure continuity and quality outcomes. The role combines remote and field nursing support with collaboration across interdisciplinary teams.

Requires an active RN license in assigned states, CPR certification, and 1+ years in hospice/home health/acute care. Schedule is Monday through Thursday, 5 PM to 8 AM, with a $7,500 sign-on bonus for eligible

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and upgrade care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
