Job summary

Amedisys, part of the Optum family, provides home health, hospice and community-based services that bring quality care to patients at home. The Hospice RN On-Call role delivers after-hours clinical support, triages needs, and coordinates care to ensure continuity and high-quality outcomes, with both remote and field-based nursing.

Responsibilities include on-call triage, patient condition assessment, coordination with physicians and care teams, and education for patients and caregivers.

Qualifications

• Current RN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
