Amedisys, part of the Optum family, is seeking a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care to ensure continuity and quality outcomes. The role delivers remote and field-based nursing support, assesses patient conditions, and collaborates with interdisciplinary teams to address urgent needs while educating patients and caregivers.

Qualificações

• Current RN license valid in the state(s) you are assigned to work.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsabilidades

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
