Job summary

Amedisys, part of the Optum family, is seeking a Registered Nurse for Hospice After Hour On-Call coverage. The role provides after-hours clinical support, triaging patient needs, and coordinating care to ensure continuity and quality outcomes across home health and hospice settings.

The RN On-Call will deliver remote and field-based nursing support, assess patient conditions, and collaborate with interdisciplinary teams to address urgent needs. Friday 5pm–Monday 8am on-call schedule.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• Valid driver's license, reliable transportation, and liability insurance.
• Current CPR certification.
• 1+ years of nursing experience, preferably in hospice, home health, or acute care.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions.
• Assess patient conditions via phone and/or in-person visits and elevate care as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
