Job summary

Amedisys, part of the Optum family, seeks a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care for continuity and quality outcomes. This role blends remote and field-based nursing to assess conditions and collaborate with care teams across the region.

Shifts are 7 days on and 7 days off in the Woburn area, with coverage for evenings and weekends. Requirements include an active RN license, CPR, and a valid driver's license, with comprehensive

Qualifications

• Current RN license in the state(s) you are assigned to work
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification
• Valid driver's license, reliable transportation, and liability insurance

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
