Job summary

Amedisys, part of the Optum family, is seeking a Hospice RN On-Call for the Woburn area to provide after-hours clinical support and triage patient needs. Responsibilities include remote and field nursing, coordinating care with interdisciplinary teams, and ensuring quality outcomes.

The role requires active RN licensure in the state, 1+ year nursing experience (hospice/home health/acute care preferred), CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Current RN license in the state(s) you are assigned to work
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification
• Valid driver’s license, reliable transportation, and liability insurance

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining appropriate interventions
• Assess patient conditions via phone and/or in-person visits and elevate care as needed
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery
• Deliver patient and caregiver education related to symptom management and care plans
• Maintain accurate and timely documentation of all patient interactions and clinical decisions
