Job summary

Amedisys, part of the Optum family, is seeking a Hospice RN On-Call to provide after-hours clinical support, triage patient needs, and coordinate care to maintain continuity and quality outcomes. This role combines remote and field-based nursing support with interdisciplinary collaboration.

Responsibilities include on-call triage by phone or in-person visits, documentation, patient and caregiver education, and coordination with physicians and care teams.

Qualifications

• Current RN license for the state(s) you are assigned to work.
• 1+ year of nursing experience, preferably in hospice, home health, or acute care
• Current CPR certification.
• Valid driver's license, reliable transportation, and liability insurance.

Responsibilities

• Provide on-call nursing support, triaging patient needs and determining interventions.
• Assess patient conditions via phone and/or in-person visits and escalate as needed.
• Coordinate with physicians, care teams, and caregivers to ensure timely and effective care delivery.
• Deliver patient and caregiver education related to symptom management and care plans.
• Maintain accurate and timely documentation of all patient interactions and clinical decisions.
