Job summary

Heartland Hospice is seeking an After Hours (on-call) Registered Nurse to respond to patient needs during evenings, nights, weekends and holidays. You will deliver compassionate care, document visits, and coordinate with the hospice team to ensure the Plan of Care is followed.

Ideal candidates hold an active RN license, CPR certification, and 1+ year of nursing experience, preferably in hospice or home-based care. A reliable vehicle and willingness to travel are required.

Qualifications

• Current RN license in the state of employment.
• 1+ years of nursing experience, preferably in hospice or home care.
• Compassionate, calm, and dependable in urgent care situations.
• Hospice and home-based care familiarity is a plus.

Responsibilities

• Provide direct patient care and support after hours.
• Serve as primary liaison between patients, families, physicians and hospice team.
• Document visits and communications in EMR.
• Assess patient needs and implement the plan of care.
• Collaborate with interdisciplinary team and participate in QA programs.
