Job summary

Elevance Health seeks a LTSS Service Coordinator – Clinician (LSW/LCSW/LPN) to support Ohio members in field settings across the state. The role involves coordinating care, performing licensure-supported assessments, and collaborating with RNs and care teams to manage physical, behavioral, and long-term needs.

The position requires relevant licensure (LPN/LVN or social work) with at least two years of experience; hybrid/field work is expected, with occasional office attendance as needed.

Qualifications

• Requires LPN/LVN, LSW, LCSW, or LMSW license in applicable state and Nursing Diploma or AS in Nursing and minimum 2 years of experience.
• Experience with chronic illnesses, co-morbidities, and disabilities in a Service Coordinator or similar role.
• BA/BS in Health/Nursing preferred.

Responsibilities

• Coordinate care for LTSS members, including assessments and care plans under RN guidance.
• Assist in identifying high-risk members and coordinating with health teams.
