Job summary

CVS Health is hiring a Community Health Worker in Ohio to bridge members with the healthcare system through outreach and education. The role requires traveling up to 75% within the region and working under professional supervision in a community setting.

The CHW will educate providers, support behavioral health goals, and help members access social services. Strong communication, cultural competency, and organizational skills are essential for success in this field-based role.

Qualifications

• Complete an approved CHW certification within 12 months of hire.
• MS Office proficiency including Outlook, Word, Excel, SharePoint.
• Experience with children/adolescents is preferred.
• Travel up to 75% within the region; reliable transportation required.

Responsibilities

• Conduct home visits with members to support care plans, adapt tasks to cultural needs, and reduce unnecessary hospitalizations.
• Educate providers on social determinants of health and care coordination opportunities.
• Build relationships with members, providers, and care teams across diverse communities.
• Help connect members with community services, including transportation and program applications.
• Assist with medication adherence by reviewing med lists and refills as needed.
