Qualifications

• Complete an approved CHW certification program within 12 months of hire; classes count as work time.
• Proficiency with computers and MS Office (Outlook, Word, Excel, SharePoint).
• Experience working with children/adolescents.
• Ability to travel up to 75% within the region; reliable transportation and driver’s license required.
• Strong communication skills to convey medical information by phone.
• Knowledge of community resources and social determinants of health.

Responsibilities

• Conduct home visits with members to support care plans and identify best options.
• Educate providers about social determinants of health affecting care and outcomes.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Help reduce barriers between clients and institutions; assist with social service access.
• Assist with medication adherence and refills, advise on alternatives when needed.
• Identify gaps in care and connect members to community services and transportation.
