CVS Health in Ohio is seeking a Community Health Worker to bridge members with the healthcare system through outreach and education. This full-time field-based role requires travel within the region and collaboration with healthcare providers to improve health outcomes.
You will educate providers on social determinants of health, support culturally appropriate care, assist with care plans, and help members navigate services and programs.
Competenze
- Complete an approved CHW certification program within 12 months of hire.
- Proficiency with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
- Experience with children/adolescent populations.
- Ability to travel up to 75% of the time to multiple office locations and member homes with reliable transportation.
Mansioni
- Conduct home visits with members to assist care plans and identify options.
- Educate healthcare professionals on social determinants of health.
- Build relationships with members, providers, and staff from diverse backgrounds.
- Work to reduce cultural and socio-economic barriers between clients and institutions.
- Assist with identification of socio-economic issues affecting overall health and development of management plans.
