Qualifications

• Complete an approved CHW certification program within 12 months of hire.
• Demonstrated proficiency with personal computers, keyboard and MS Office Suite.
• Experience with children/adolescent populations.
• Ability to travel up to 75% within the region and maintain reliable transportation.
• Flexible to work in varied home environments and cultures.
• Ability to communicate medical/mental health information over the phone.
• Knowledge of community resources and multi-system outreach for health care.

Responsabilités

• Conduct home visits to assist members with care plans and identify best options.
• Educate providers on social determinants of health affecting care and treatment.
• Build relationships with members, providers, case managers, and staff.
• Work to reduce barriers between clients and institutions.
• Assist with identifying socio-economic issues impacting health and goals.
• Identify gaps to strengthen care systems within the member's community.
• Assist with medication adherence by reviewing and coordinating refills.
