Job summary

CVS Health is hiring a Community Health Worker in Ohio to bridge members with the healthcare system. The role requires up to 75% travel within the region and entails home visits, education on social determinants of health, and coordination with diverse care teams.

The CHW will support care plans, assist with accessing services, and help members navigate treatment and appointments. A CHW certification is required within 12 months of hire, with strong communication and organizational skills

Qualifications

• Complete an approved CHW certification program within 12 months of hire.
• Demonstrated proficiency with personal computers and MS Office Suite.
• Experience with children/adolescent populations.
• Ability to travel up to 75% of the time to multiple locations; reliable transportation required.
• Flexibility to work in variable environments and communicate medical information clearly.

Responsibilities

• Conduct home visits with members to support care plans and identify options.
• Educate providers on social determinants of health to improve care.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Assist members in accessing community services and completing applications.
• Support medication adherence by reviewing medications with members.
