CVS Health in Ohio seeks a Community Health Worker to bridge members with the healthcare system through outreach and education. The role requires up to 75% travel within the region, and involves home visits, coordinating services, and supporting medication adherence.

The CHW works under supervision, engages diverse communities, and helps minimize barriers to care. ACHW certification within 12 months is expected, with strong MS Office skills and excellent communication essential.

Competenze

• Complete a CHW certification within 12 months of hire.
• Proficiency with MS Office Suite (Outlook, Word, Excel, SharePoint).
• Experience with children/adolescent populations.
• Must have reliable transportation and valid driver's license.
• Ability to travel up to 75% within the region.
• Strong interpersonal and communication skills.

Mansioni

• Conduct home visits with members to support care plans and identify best options considering culture, language, and religion.
• Educate providers on social determinants of health affecting care and treatment plans.
• Build relationships with members, providers, and staff from diverse backgrounds.
• Work to reduce cultural and socio-economic barriers between clients and institutions.
• Assist members in identifying socio-economic issues and developing health/social plans.
• Identify gaps and help navigate community services and social program applications.
• Assist with medication adherence by reviewing current lists and obtaining refills.
