CVS Health is seeking a Community Health Worker in Ohio to bridge members with the healthcare system through outreach and education. The role requires up to 75% travel, field-based work, and collaboration with diverse teams to improve health outcomes.

You will support care plans, educate on social determinants of health, and assist with access to community services, appointments, and medications. A CHW certification is required within 12 months of hire.

Competenze

• Complete CHW certification within 12 months of hire; time counted as work time.
• Proficiency with computers, multiple systems, and MS Office (Outlook, Word, Excel, SharePoint).
• Experience with children/adolescent populations.
• Willingness to travel up to 75% across the region; own vehicle and license required.
• Ability to communicate medical/mental health information to health professionals by phone.
• Knowledge of community resources and services.

Mansioni

• Visit homes with members to support care plans and identify optimal options.
• Educate providers about social determinants of health and engage teams across channels.
• Build relations with members, providers, and community staff from diverse backgrounds.
• Help reduce barriers between clients and institutions and assist with program applications.
• Support medication adherence by reviewing lists and assisting with refills.
