Job summary

CVS Health is seeking a Community Health Worker to serve Ohio communities. The role involves field-based outreach, home visits, and collaboration with healthcare teams to improve health outcomes.

You will educate members on care plans, assist with access to services, and support medication adherence while navigating diverse cultural contexts. A CHW certification and strong communication skills are essential for success.

Qualifications

• Complete an approved CHW certification program within 12 months of hire.
• Proficient with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• Experience with children/adolescent populations.
• Ability to travel up to 75% of the time to multiple locations; reliable transportation required.
• Strong communication and interpersonal skills with diverse communities.
• Ability to work in non-traditional settings and adapt to varying home environments.

Responsibilities

• Conduct home visits to assist members with care plans and identify options that best meet needs.
• Educate providers on social determinants of health affecting care plans.
• Build and maintain positive relationships with members, providers, case managers, and staff.
• Identify and address cultural, linguistic, and socio-economic barriers to care.
• Assist with scheduling appointments, transportation, and applications for programs.
• Support medication adherence by educating members on current lists and refills.
