Job summary

Qualifications

• Complete a CHW certification within 12 months of hire (training provided/reimbursed).
• Proficient with MS Office and multiple computer systems (Outlook, Word, Excel, SharePoint).
• Eligible to travel up to 75% within the region; valid driver's license and insured vehicle required.
• Strong communication and interpersonal skills; ability to work with diverse populations.
• Must be flexible in variable home environments and working conditions.

Responsibilities

• Conduct home visits with members to support care plans and needs.
• Educate providers about social determinants of health affecting care.
• Build relationships with members, providers, and community resources.
• Help identify barriers and connect members to community services and programs.
• Assist with medication adherence and coordination of refills as needed.
