Job summary

Innovate-Rehab-and-Wellnes in Raytown, MO is seeking a PRN Certified Occupational Therapy Assistant to deliver one-on-one therapy in our Skilled Nursing Facility. You will implement therapy plans and work closely with OT and nursing staff to support residents’ rehab goals.

Qualifications

• Associate’s degree from an accredited OTA program.
• NBCOT certification is required.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Committed to patient-centered care and rehab excellence.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
