Job summary

INNOVATE REHAB AND WELLNESS in Natchitoches, LA seeks a Licensed PRN Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility setting. The role focuses on delivering high-quality OT services under supervision, helping residents regain independence and improve quality of life.

The flexible, as-needed schedule supports work-life balance, with one-on-one patient care and collaboration with the care team to achieve strong clinical outcomes and patient satisfaction.

Qualifications

• Associate degree in OTA from an accredited program.
• Current NBCOT certification.
• Valid state OTA license.
• SNF or long-term care experience preferred.
• Excellent communication and teamwork.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
