Job summary

Innovate Rehab and Wellness in Sedalia, MO is seeking a Licensed PRN Certified Occupational Therapy Assistant to join our Skilled Nursing Facility. You will implement treatment plans, provide direct patient care, and coordinate with the care team to support resident outcomes and safety.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
