Job summary

Innovate Rehab and Wellness is seeking a licensed Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility. The role offers flexible, as-needed PRN shifts with a focus on one-on-one patient care and collaborative interprofessional work.

The COTA will deliver high-quality OT services under supervision, help residents regain independence, and support care planning and documentation in a SNF setting.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an OTA.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
