Job summary

INNOVATE REHAB AND WELLNESS is seeking a licensed PRN Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility position in St. Elizabeth, MO.

You will deliver high-quality OT services, help residents regain independence, and work under OT supervision in a collaborative team environment. The role emphasizes one-on-one therapy, with group/concurrent treatments under 1%, and a realistic schedule to support work-life balance.

Qualifications

• Associate degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state OTA license.
• SNF/long-term care experience preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and in a team.

Responsibilities

• Support delivery of high-quality OT services.
• Help residents regain independence and improve quality of life.
• Provide therapeutic interventions under OT supervision.
• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
