Job summary

Innovate Rehab and Wellness in Trenton, MO is seeking a licensed COTA to provide high-quality occupational therapy in a skilled nursing facility. This PRN role supports residents in regaining independence and improving quality of life, with flexible scheduling to fit work–life balance.

Under OT supervision you will implement treatment plans, document progress in the EMR, educate residents and caregivers, and collaborate with nursing, PT, SLP and other staff to ensure patient-centered rehab

Qualifications

• Associate’s degree from an OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.
• Committed to patient-centered care and rehab excellence.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
