INNOVATE REHAB AND WELLNESS in Kansas City, MO is seeking a licensed Occupational Therapy Assistant (COTA) to support delivery of high-quality OT services for residents in a skilled nursing facility. You will help residents regain independence and improve quality of life under OT supervision.

The role emphasizes one-on-one therapy, collaboration with nursing, PT, SLP, and other staff, and participation in care and discharge planning.

Qualifications

• OTA degree and NBCOT certification required.
• Active state license as OTA required.
• Ability to work in a skilled nursing facility environment.

Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations
