Job summary

Innovate Rehab and Wellness in Brunswick, MO is seeking a licensed Certified Occupational Therapy Assistant (COTA) for PRN, Skilled Nursing Facility setting. You will support delivery of high-quality OT services and help residents regain independence.

Responsibilities include implementing treatment plans, documenting progress, collaborating with nurses and therapists, and ensuring HIPAA compliance. A strong communicator with NBCOT certification and state license preferred, with SNF experience.

Qualifications

• Associate’s degree from an accredited OTA program.
• Current NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
