Job summary

Innovate Rehab and Wellness in Raytown, MO is seeking a PRN Certified Occupational Therapy Assistant for a skilled nursing facility. The role supports flexible, as-needed schedules and focuses on one-on-one patient care.

You will implement treatment plans, document progress, and collaborate with the care team while adhering to HIPAA, CMS, and state guidelines. Requires OTA degree, NBCOT, and MO license.

Qualifications

• Associate’s degree from an OTA program or equivalent.
• NBCOT certification current.

• SNF or long-term care experience preferred.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OT.
• Educate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
