Innovate Rehab and Wellness is seeking a Licensed PRN Certified Occupational Therapy Assistant in Milan, MO. This PRN role allows for flexible scheduling to enhance work-life balance while providing high-quality therapy services.

The ideal candidate will support residents in regaining independence and improving their quality of life through therapeutic interventions, collaborating effectively with a supportive, therapist- and veteran-owned team. Experience in skilled nursing facilities is preferred.

Position Summary

As a COTA, you will:

• Support delivery of high-quality OT services
• Help residents regain independence and improve quality of life
• Provide therapeutic interventions under OT supervision

Key Responsibilities

• Implement treatment plans created by the Occupational Therapist
• Provide direct patient care per state and facility guidelines
• Monitor and document resident progress; report changes to the OT
• Educate residents and caregivers on adaptive techniques and equipment
• Collaborate with nursing, PT, SLP, and other healthcare staff
• Maintain accurate, timely documentation in the EMR
• Participate in care planning and discharge planning
• Ensure compliance with HIPAA, CMS, and all regulations

Qualifications

• Associate’s degree from an accredited OTA program
• Current NBCOT certification
• Valid state license as an Occupational Therapy Assistant
• SNF or long-term care experience preferred
• Strong communication and interpersonal skills
• Able to work independently and as part of a team
• Committed to patient-centered care and rehab excellence
