Job summary

Innovate Rehab and Wellness is seeking a licensed Certified Occupational Therapy Assistant (COTA) for a PRN role in our Skilled Nursing Facility in Sedalia, MO. You will provide direct patient care and help residents regain independence under OT supervision, with a focus on patient-centered rehab.

Qualifications

• Associate’s degree from an accredited OTA program and NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills; ability to work independently and in a team.

Responsibilities

• Deliver high-quality OT services under supervision of an OT.
• Assist residents to regain independence and improve quality of life.
• Provide direct patient care and implement treatment plans.
• Monitor resident progress and document changes in EMR.
• Collaborate with nursing, PT, SLP, and other staff.
• Participate in care planning and discharge planning.
• Ensure HIPAA, CMS, and facility guidelines compliance.
