Job summary

Innovate Rehab and Wellness in Missouri is seeking a Licensed PRN Certified Occupational Therapy Assistant (COTA) for a Skilled Nursing Facility. Flexible, as-needed shifts support work-life balance while delivering focused, one-on-one patient care under OT supervision.

You will implement treatment plans, help residents regain independence, document progress in the EMR, and collaborate with nursing, PT, SLP, and other staff.

Qualifications

• Associate’s degree from an accredited OTA program.
• NBCOT certification.
• Valid state license as an Occupational Therapy Assistant.
• SNF or long-term care experience preferred.
• Strong communication and interpersonal skills.
• Able to work independently and as part of a team.
• Committed to patient-centered care and rehab excellence.

Responsibilities

• Implement treatment plans created by the Occupational Therapist.
• Provide direct patient care per state and facility guidelines.
• Monitor and document resident progress; report changes to the OTEducate residents and caregivers on adaptive techniques and equipment.
• Collaborate with nursing, PT, SLP, and other healthcare staff.
• Maintain accurate, timely documentation in the EMR.
• Participate in care planning and discharge planning.
• Ensure compliance with HIPAA, CMS, and all regulations.
